CANCELLATIONS
If you need to cancel or change an appointment, please contact our office to
do so at least 24 hours before your scheduled appointment. Missed appointment
fees will be assessed as follows: for missed office visits, the “no-
show” fee is $30.00; for missed surgery appointments, the “no-show” fee
is $60.00.
REFERRALS OR AUTHORIZATIONS
If your health plan requires a referral or authorization for specialty services,
please have your primary care physician fax at 856-232-7506 or obtain through
insurance web-site a referral (paper or electronic) before your scheduled
appointment. If a referral is required and not in our office at the time
of your appointment, you may either
(1) pay for the visit at the time of service, or
(2) change your appointment to a later date so you may obtain the
referral/authorization.
COPAYMENT
It is our policy to collect payment at the time of service. If you do not have
this payment available at time of check-in, we reserve the right to reschedule
your appointment. For your convenience we accept personal local checks (with
a valid identification card), cash, and credit/debit cards (Visa and MasterCard).
INSURANCE CARD
If you are a member of one of our participating insurance companies, we require
you present your insurance card at the time of service. Please

RETURNED CHECK FEE
Any returned check will result in a $30.00 penalty fee.
REFILLS
Prescription refill request often require an office visit before we can fill
the refill request; this is to ensure your safety. Should you desire a prescription
refill, please have your pharmacist fax the request to us at
